RN with recent Telemetry and/or cardiac experience.
Disease management position to telephonically manage heart failure patients.
Will work with Cardiologist, Cardiac Nurse Practitioner, and Physician
Assistant.
Duties:
* Educate in disease process, medications, and diet; assist and
support patients/caregivers t help self manage their disease.
* Utilizing bio-metric scales monitor and manage disease process.
* Manage medications
* Manage symptoms
* Coordinate services
Goal: Assist patients to become knowledgeable about their disease
process, recognize S&S.
REqires:
Must have comfortable knowledge of cardiac medications.
Excellent teaching skills
Strong organizational skills
Ability to prioritize
Computer literate.
Case management experience and CCM preferable, but not necessary.
